left of the screen and download, print, and fill out all necessary
forms, then bring your completed paperwork with you to your
appointment.
Appointments & Office Information For
your convenience we have multiple locations to serve you. (See Locations
Page.)
Sometimes a patient may need to be seen at a different office
because it offers a specialized procedure. Be sure to confirm the
location as well as the time of your appointment.
Our goal is to schedule appointments so each patient receives
thorough, personalized care with minimum waiting time. Occasionally,
delays are unavoidable. Should a delay occur, we will try to contact
you to see if you would prefer to reschedule your appointment.
When you are unable to keep an appointment, please give us 24
hours notice so we can assign that time to another patient. That
will let us adjust our schedules so other patients do not have to
wait.
Referrals Please understand how your
insurance policy works. It is best to assume that your plan requires
a referral, unless otherwise stated. It is your responsibility to
obtain a referral number from your primary care physician. Bring
your referral numbers to each visit.
These referrals have a limited number of visits and an expiration date (usually 60-90 days from
date of issue). If you are to continue seeing a specialist beyond
the allowed number of visits, you must request another
referral from your Primary Care Physician.
Emergencies If you have an emergency after
office hours, call our office at 404-294-4018 or 770-922-2217. Our
answering service will immediately contact the physician on call. If
an extreme emergency occurs, go directly to the nearest emergency
room. |
